Способ применения и дозы

Редко — головокружение, крайне редко — крапивница, отек Квинке, отек лица и экзантема.
Передозировка
Симптомы: повышение возбудимости ЦНС (теоретически).

Фармакодинамика
Спазмолитик миотропного действия, оказывает прямое действие на гладкую мускулатуру ЖКТ (главным образом толстого кишечника). Устраняет спазм, не влияя на нормальную перистальтику кишечника. Не обладает антихолинергическим действием.
Фармакокинетика
При приеме внутрь подвергается пресистемному гидролизу и не обнаруживается в плазме. Метаболизируется в печени до вератровой кислоты и мебеверинового спирта. Выводится главным образом почками в виде метаболитов, в небольших количествах — с желчью. Капсулы мебеверина имеют свойства продолжительного высвобождения. Даже после многократного приема не наблюдается значительной кумуляции.

Описание лекарственного препарата МОНОКАПС РЕТАРД основано на официально утвержденной инструкции по применению препарата и сделано в 2007 году. Дата обновления: 22.12.2006 г.
Фармакологическое действие
Антиангинальный препарат. Изосорбида мононитрат — периферический вазодилататор с преимущественным влиянием на венозные сосуды.
Увеличивает содержание оксида азота в эндотелии, стимулирует гуанилатциклазу и образование цГМФ, уменьшает концентрацию кальция в клетках гладких мышц. Вызывает уменьшение потребности миокарда в кислороде за счет уменьшения преднагрузки (расширение периферических вен и уменьшение притока крови к правому предсердию) и постнагрузки (уменьшение ОПСС), а также оказывает коронарорасширяющее действие. Улучшает коронарный кровоток, способствует его перераспределению в ишемизированные области, уменьшает конечный диастолический объем левого желудочка и снижает систолическое напряжение его стенок. Повышает толерантность к физической нагрузке у больных с ИБС, стенокардией. Снижает давление в малом круге кровообращения.
Активное вещество Монокапса ретард состоит из 2 частей, одна из которых действует непосредственно после приема препарата, другая – постепенно в течение длительного времени. Таким образом, препарат действует быстро и длительно в отношении предупреждения приступов стенокардии.
Фармакокинетика
Всасывание
После приема внутрь изосорбида мононитрат быстро и полностью абсорбируется из ЖКТ. Биодоступность составляет 90-100% и не зависит от особенностей диеты (калорийность, высокое или низкое содержание жиров) и времени приема пищи. Концентрация в плазме крови прямо пропорциональна принятой дозе. Tmax составляет 8 ч.
Метаболизм
Изосорбида мононитрат почти полностью биотрансформируется в печени с образованием неактивных метаболитов путем денитрации до изосорбида или посредством конъюгации с глюкуроновой кислотой и образованием неактивного 2-глюкуронида.
Выведение
Выводится почками преимущественно в виде метаболитов, 2% — в неизменном виде.
Фармакокинетика в особых клинических случаях
Фармакокинетика изосорбида мононитрата не изменяется у лиц пожилого и старческого возраста, а также у больных с хронической почечной и печеночной недостаточностью.
Показания к применению
- профилактика и лечение стенокардии III-IV функционального класса;
- постинфарктный период;
- при спазме периферических артерий (облитерирующий эндартериит, ангиоспастический ретинит);
- в составе комбинированной терапии при лечении хронической сердечной недостаточности, некоторых формах легочной гипертензии и легочного сердца.

Режим дозирования
Монокапс ретард принимают внутрь по 1 капсуле утром, после завтрака, запивая небольшим количеством воды.
Частота приема и длительность лечения устанавливаются индивидуально, зависят от показаний и эффективности проводимой терапии и определяются лечащим врачом.
Побочные действия
Со стороны сердечно–сосудистой системы: в начале лечения возможна «нитратная» головная боль (обычно исчезающая через несколько дней при продолжении лечения); возможна артериальная гипотензия (в т.ч. ортостатическая), сопровождающаяся рефлекторной тахикардией, заторможенностью, головокружением, слабостью; редко при выраженной артериальной гипотензии — усиление симптомов стенокардии (парадоксальное влияние нитратов); в отдельных случаях – коллапс, брадиаритмия, синкопе.
Со стороны пищеварительной системы: редко – тошнота, рвота.
Дерматологические реакции: возможно транзиторное покраснение кожи; в отдельных случаях – эксфолиативный дерматит.
Аллергические реакции: возможны кожные проявления.
Прочие: в отдельных случаях – снижение скорости психомоторных реакций.
Противопоказания к применению
- острые нарушения кровообращения (шок, сосудистый коллапс);
- геморрагический инсульт;
- острый инфаркт миокарда с пониженным давлением наполнения левого желудочка;
- выраженная артериальная гипотензия (систолическое АД менее 90 мм рт.ст., диастолическое АД – менее 60 мм рт.ст.);
- гипертрофическая обструктивная кардиомиопатия;
- конструктивный перикардит;
- тампонада сердца;
- кровоизлияние в мозг;
- первичная легочная гипертензия;
- глаукома;
- детский и подростковый возраст до 18 лет;
- повышенная чувствительность к органическим нитратам.
Особые указания
Монокапс ретард не применяют для купирования острых приступов стенокардии, острого инфаркта миокарда.
С осторожностью назначают пациентам с глаукомой, при повышенном внутричерепном давлении, аортальном и/или митральном стенозе, склонности к артериальной гипотензии, при нарушениях мозгового кровообращения и выраженных нарушениях функции почек.
При длительном применении возможно развитие толерантности, в связи с чем рекомендуется после 3-6 недель регулярного приема препарата делать перерыв на 3-5 дней, заменив на это время Монокапс ретард другими антиангинальными препаратами. При приеме препарата может возникнуть временная гипоксемия за счет относительного перераспределения кровотока в гиповентилируемые альвеолярные зоны. У пациентов с ИБС это может привести к временной гипоксии миокарда.
В период лечения Монокапсом ретард следует исключить употребление алкоголя.
Влияние на способность к вождению автотранспорта и управлению механизмами
При приеме препарата возможно снижение способности к концентрации внимания и скорости психомоторных реакций, в связи с чем не рекомендуется управление транспортными средствами и занятия другими потенциально опасными видами деятельности.
Передозировка
Симптомы: выраженное снижение АД, коллапс, рефлекторная тахикардия, головная боль, слабость, головокружение, гиперемия кожи, тошнота, рвота и диарея. Большие количества нитратов могут вызвать метгемоглобинемию, цианоз, одышку и тахиапноэ.
При очень высоких дозах препарата может повышаться внутричерепное давление с церебральными симптомами. При хронической передозировке выявляется повышенный уровень метгемоглобина.
Лечение: промывание желудка. При выраженном снижении АД и/или шоке следует провести плазмозамещающую терапию.
Применение эпинефрина (адреналина) и родственных ему соединений противопоказано.
При метгемоглобинемии: аскорбиновая кислота – 1 г в/в, 1% раствор метилтиониния хлорида (метиленовый синий) – 1-2 мг/кг (до 50 мл) в/в, оксигенотерапия, гемодиализ, переливание крови.
Лекарственное взаимодействие
При одновременном применении Монокапса ретард с антигипертензивными средствами, бета-адреноблокаторами, блокаторами кальциевых каналов, ингибитором фосфодиэстеразы – сильденафилом возможно усиление антигипертензивного действия.
Антигипертензивный эффект усиливают также нейролептики, трициклические антидепрессанты и этанол.
При одновременном применении Монокапса ретард и дигидроэрготамина возможно усиление гипертензивного действия и повышение концентрации дигидроэрготамина в плазме крови.
При одновременном применении Монокапса ретард и норэпинефрина (норадреналина) терапевтический эффект последнего уменьшается.
Барбитураты ускоряют биотрансформацию и снижают концентрацию изосорбида мононитрата в плазме крови.
Адсорбенты, вяжущие и обволакивающие средства уменьшают всасывание изосорбида мононитрата из ЖКТ.
Условия хранения препарата
Список Б. Препарат следует хранить в недоступном для детей, сухом, защищенном от света месте при температуре от 15° до 25°С.

Uses
For the symptomatic treatment of Irritable Bowel Syndrome (IBS) and other conditions include:

Dosage
135 mg tid, may reduce gradually after several wk when the desired effect has been obtained; 100 mg tid has also been used. As modified-release cap: 200 mg bid.
Colospa Retard is most effective when taken 20 minutes before meals. After several weeks when the desired effect has been obtained, the dosage may be gradually reduced.
Missed dose: If a dose of this medicine is missed, that should be taken as soon as possible. However, if it is almost time for the next dose, then skip the missed dose and the regular dosing schedule should be maintained. Dose should not be doubled at the same time to compensate the missed dose.
Side Effects
Generally Colospa Retard is well tolerated. However, few side-effects like skin rash, urticaria and angioedema may appear.
Precaution
Caution should be exercised in porphyria or allergic reaction to this or any other medicine of this group.

Pregnancy & Breastfeeding use
No teratogenicity has been shown in animal experiments. However, the usual precautions concerning the administration of any drug during pregnancy should be exercised. Colospa Retard does not excrete in the breast milk after administering the therapeutic dose.

Use in children: Colospa Retard is not recommended in children under 10 years.
